Substance use affects people of all ages and walks of life across Iowa. In recent years, overdose rates have risen steadily, particularly from opioids and methamphetamine. According to the Iowa Department of Public Health, in 2022, Iowa reported more than 470 drug overdose deaths—a 40% increase over the past five years. While Iowa has one of the lower overdose rates in the nation, the trend is moving in the wrong direction.

Opioids remain a serious concern. Nearly 80% of Iowa’s overdose deaths involved opioids, with synthetic opioids like fentanyl leading the charge. Fentanyl is especially dangerous due to its potency and its increasing presence in counterfeit pills and mixed substances. Methamphetamine use is also widespread in both rural and urban communities, accounting for more than 25% of all Iowa treatment admissions.

Alcohol misuse continues to have a major impact on the state’s health as well. Iowa has one of the highest binge drinking rates in the country with over 21% of adults reporting binge drinking at least once in the past month, significantly higher than the national average. College towns and rural areas alike are struggling with the consequences, from liver disease to impaired driving.

Youth substance use also deserves attention. While tobacco and alcohol use among Iowa teens has declined slightly, vaping and marijuana use are on the rise. Nearly 30% of 11th graders in Iowa reported using e-cigarettes in the past month, and about 16% reported using marijuana.
